In Fixing Those 9,553 EHR "Issues", Southern Arizona’s Largest Health Network is $28.5 Million In The Red

That's the usual managerial solution for financial losses, and in this case I feel particularly worried about employees who did not obey the "suggestion" to only use "<b>Words that Work</b>" about this EHR (see <a href="http://hcrenewal.blogspot.com/2013/10/words-that-work-singing-only-positive.html">http://hcrenewal.blogspot.com/2013/10/words-that-work-singing-only-positive.html</a>). <br /><br /><blockquote class="tr_bq"><div style="margin-bottom: 0in;">The electronic records system, from Wisconsin-based Epic Systems, has cost an estimated <b>$115 million</b>, including <b>$32 million in unbudgeted costs for the first eight months of the fiscal year,</b> which ends June 30, financial documents show. The extra costs are due primarily to a delay in getting the system live and funding additional training and support, officials said. It was supposed to be up and running by Sept. 1, but wasn’t operational until Nov. 1.</div></blockquote><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">$32 million to fix 9,553 "issues", most likely due to implementation incompetence, plus some that perhaps the seller should have at least been partially responsible for...such as these:</div><div style="margin-bottom: 0in;"><br /></div><blockquote class="tr_bq"><ul><li>Pharmacy Medication Mapping Errors <span style="color: red;"></span></li></ul><ul><li>Microbiology Results Mapping Incorrectly </li></ul></blockquote><blockquote class="tr_bq"><ul><li>Prescription printing bugs</li></ul><ul><li>Errors transmitting prescriptions </li></ul></blockquote><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">I note that real, live patients were the unwitting experimental subjects in this little software debugging project.</div><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">I also note that $115 million <b>could have paid for an entire new hospital or wing</b> - and these startup costs are just the beginning... </div><div style="margin-bottom: 0in;"><br /></div><blockquote class="tr_bq"><div style="margin-bottom: 0in;">... There are other reasons for the network’s weak finances. UA Health Network officials say uninsured patients aren’t getting health insurance coverage through the federal Affordable Care Act as quickly as they’d anticipated, resulting in $11 million less than expected in patient revenue through February.</div><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">The network also lost federal dollars from a program that helped bridge the funding gap created by Medicaid rate cuts and a rise in uninsured patients. The Safety Net Care Pool, which also included Maricopa Integrated Health System and Phoenix Children’s, pumped $33 million into the UA hospitals for the first six months of the fiscal year. But that infusion ended Dec. 31 to coincide with greater health insurance availability through the Affordable Care Act.</div></blockquote><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">It's not like these events were entirely unpredictable.  Perhaps priorities could have been rearranged, rather than dumping millions into a known money pit, the implementation of enterprise clinical information technology?</div><blockquote class="tr_bq"><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">... “The issue is more about where we are going and what things are in place to change the trajectory,” Lynn said. “It was an especially difficult time financially because of Epic, there was no choice in the matter. That period of time has fortunately passed and now we can do much better.”</div></blockquote><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">There certainly was "choice in the matter."  I also suggest to Mr. Lynn that the "trajectory" might end up being a ballistic one.</div><div style="margin-bottom: 0in;"><br /></div><blockquote class="tr_bq"><div style="margin-bottom: 0in;">... A financial report presented to the network’s board of directors on April 24 says the Epic system’s higher-than-expected expenses this fiscal year were due to “implementation delays, additional training support and planned schedule reductions.” Some of the expenses were originally supposed to be in the prior fiscal year.</div><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">The report attributes $6.8 million of the current year’s losses to physicians spending enough time training to use the new system that they couldn’t see as many patients between November and January. Schedules were back to normal as of February.</div></blockquote><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">One wonders just how much increased stress and pressure there is on clinicians with the new system in place.   </div><div style="margin-bottom: 0in;"><br /></div><blockquote class="tr_bq"><div style="margin-bottom: 0in;">... Implementing the new system has hit the network’s finances hard, but over time it <b>should improve patient safety</b> by cutting down on unnecessary testing and medication errors, among other things, officials say. We have to have electronic medical records,” said Dr. Kwan S. Lee, a cardiologist who is medical director of the UA Medical Center’s south campus.</div></blockquote><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">"This is the future!!  We just <i><b><u>have</u> </b></i>to do it!!"  Dr. Lee, I admire your exceptionally well-reasoned and thorough budget justification for putting your healthcare system $30 million in the red.  Congratulations!</div><div style="margin-bottom: 0in;"><br /></div><blockquote class="tr_bq"><div style="margin-bottom: 0in;">... Lee cited a recent case where a patient who had been previously treated at Tucson Medical Center came to the UA Medical Center’s emergency room, having an acute heart attack. The standard of care in such cases is opening the artery within 90 minutes. In a rush against the clock, Lee was able to click on the patient’s record from TMC and see the area where the patient had prior surgery.</div><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">“The patient had consented to share information from Tucson Medical Center and it helped us immensely,” Lee said. “We would have possibly wasted a lot of time. The patient did very well.”</div></blockquote><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">A far, far less expensive document image management system could have accomplished exactly the same goal.</div><div style="margin-bottom: 0in;"><br /></div><blockquote class="tr_bq"><div style="margin-bottom: 0in;">One con is that the system is <b>“a little overly complicated”</b> because it tries to be all things to all people, he said.  “It was painful ... <b>A lot of us are technophobes,</b>” he said. “But there was no way we could move forward without adopting it.” </div></blockquote><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">"A little overly complicated?"  Gee, I guess that's why physicians at Athens Regional Medical Center revolted as at <a href="http://hcrenewal.blogspot.com/2014/05/i-could-not-make-this-up-if-i-tried.html">http://hcrenewal.blogspot.com/2014/05/i-could-not-make-this-up-if-i-tried.html</a>.  The system was just "a little overly complicated!"</div><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">"A lot of us are technophobes?"  How about "A lot of us are not EHR hyper-enthusiasts, but pragmatists?" (<a href="http://hcrenewal.blogspot.com/2012/03/doctors-and-ehrs-reframing-modernists-v.html">http://hcrenewal.blogspot.com/2012/03/doctors-and-ehrs-reframing-modernists-v.html</a>).</div><div style="margin-bottom: 0in;"><br /></div><blockquote class="tr_bq"><blockquote class="tr_bq"><div style="margin-bottom: 0in;">Chief Information Officer Critchley agreed, saying that with the infrastructure in place, the system will be improved as needed over time.</div><div style="margin-bottom: 0in;"><b><br /></b></div><b></b><br /><div style="margin-bottom: 0in;"><b>“For better or worse</b>, only when it’s live are you going to get the doctors and staff to engage and do the fine tuning <b>of what it really means</b>, and what they want to see happen,” Critchley said.</div></blockquote></blockquote><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">This is a political statement at best.  The assumption here is that this technology itself is some sort of miracle worker, just needing "fine tuning" to reveal its miracles.  In fact, most medical inefficiencies and dangers have nothing to do with documentation at all (e.g., see "<b>Is healthcare IT a solution to the wrong problem?</b>" at <a href="http://hcrenewal.blogspot.com/2010/12/is-healthcare-it-solution-to-wrong.html">http://hcrenewal.blogspot.com/2010/12/is-healthcare-it-solution-to-wrong.html</a>).</div><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;"><b>The only miracle so far has been to put the system $30 million in the red</b>, with more to follow.</div><blockquote class="tr_bq"><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">... The UA Health Network, which began its Epic implementation in 2012, receives a little over $1 million per year in incentive payments, Critchley said.</div></blockquote><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">That's nice.   They spent $115 million to qualify for a $1 million annual incentive?  Makes perfect sense to me ...</div><div style="margin-bottom: 0in;"><br /></div><blockquote class="tr_bq"><div style="margin-bottom: 0in;"><b>Adoption of electronic records is voluntary</b>, but hospitals that don’t make the switch by October face Medicare payment cuts of about 1 percent, and increasing reductions each year.</div></blockquote><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">At least one reporter reports this factoid accurately.  Hospital and industry execs are fond of misstatements that "EHRs are mandatory."</div><blockquote class="tr_bq"><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">Critchley has worked on at least four other Epic installations around the country as part of his consulting work and says <b>the UA Medical Center’s has been the least expensive</b>, with the most aggressive timeline. He has worked on projects that cost more than <b>a half billion dollars</b>.</div></blockquote><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">"Least expensive?"  "Half a billion dollars?"  It sounds like first class care for every underserved person in the United States could have been paid for instead of dumping the money into an unproven, experimental computer technology...</div><div style="margin-bottom: 0in;"><br /></div><blockquote class="tr_bq"><div style="margin-bottom: 0in;">At Tucson Medical Center, a $31 million upgrade to its electronic medical records system — also the Epic system — began to reduce expenses <span style="color: red;"><i><b>[overall net expenses, or just on a limited per-account, cherry-picked basis?  I suspect the latter - ed.]</b></i></span> almost immediately after it went live in 2010. TMC is among 3 percent of hospitals nationwide at the highest stage of electronic records adoption, based on an electronic records ranking system.</div><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">“We started to see efficiencies the first day,” said Frank Marini, TMC’s chief information officer. <b>Transcription expenditures dropped by half, as doctors who had previously dictated patient notes instead documented them directly into the electronic record, he said.</b></div></blockquote><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">And they had to type and/or manually edit the results of Dragon or some other VR software.  That's wonderful!  Save money by firing transcriptionists and <i>making the physicians spend their valuable time and attention on clerical work.</i>  Fine business!</div><blockquote class="tr_bq"><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">TMC began its push to upgrade its partial electronic records system in 2009. The hospital’s early investments in infrastructure, starting in 2002, lowered the cost of adopting a comprehensive system, he said.</div></blockquote><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">Thus, in fact, overall expense was probably <i><b>several times the $115 million quoted.  Say, perhaps, half a billion dollars?</b></i></div><div style="margin-bottom: 0in;"><br /></div><div style="margin-bottom: 0in;">To any U. Clouded "Visionary" Leadership - Wake Forest Baptist Medical Center's EPIC "Business Cycle Disruptions"

A typical excuse for the multi-million dollar compensation now enjoyed by many leaders of health care organizations is these leaders' supposed brilliance.<br /><br />For example, in 2011 we <a href="http://hcrenewal.blogspot.com/2011/05/million-dollar-plus-hospital-ceo.html">noted  </a>that the total compensation of Dr John McConnell, the CEO of Wake Forest Baptist Medical Center, a non-profit teaching hospital, rose from over <b><i>$700,000</i></b> in 2008-2009 to over <b><i>$1.6 million</i></b> in 2009-2010.  Other top executives in the system made nearly one million a piece.  An official statement from the hospital system claimed that this level of compensation was needed to "retain skilled executives and <b><i>visionary leaders</i></b> for the medical center."  Furthermore, in 2012 we <a href="http://hcrenewal.blogspot.com/2012/05/more-rising-compensation-for-executives.html">noted </a>that in 2010-2011 Dr McConnell's compensation had grown to nearly <b><i>$2.5 million</i></b>, while other top executives received from over $900,000 to over $1.1 million.  <br /><br />Recent events, however, suggest that the "visionaries" may need new glasses.<br /><br /><b>An EPIC Challenge </b><br /><br />Last month, the Winston-Salem Journal <a href="http://www.journalnow.com/business/business_news/local/article_c2801866-9e0c-11e2-bf84-0019bb30f31a.html">reported</a> that Wake Forest Baptist Medical Center is facing some unexpected fiscal challenges, especially from its new electronic health record (EHR):<br /><br /><span class="paragraph-0"></span><br /><blockquote class="tr_bq">Wake Forest Baptist Medical Center’s <i>struggles to implement its <a href="http://hcrenewal.blogspot.com/search/label/EPIC">Epic electronic records system</a></i> <i>contributed to additional costs and lost revenue</i> during the first half of its fiscal year 2012-13.<br /><span class="paragraph-1"> <br />The center provided the information in a second-quarter financial report submitted to bond agencies in which it also reported <i>a $49.6 million operational loss</i> and a gain of $7.4 million in overall excess revenue.<br /> </span></blockquote><br />That is interesting.  There have been many criticisms of EHRs, particularly for how they may <a href="http://hcrenewal.blogspot.com/search/label/healthcare%20IT%20difficulties">impede</a>, rather than help health professionals, and more importantly for their risks of causing <a href="http://hcrenewal.blogspot.com/search/label/healthcare%20IT%20dangers">adverse effects</a> affecting patients, in the absence of clear data from controlled clinical trials that they provide benefits that outweigh their potential harms to patients.  Some of these problems may stem from design and implementation that prioritizes benefits to managers and institutional finances over effects on patients and doctors.  As InformaticsMD <a href="http://hcrenewal.blogspot.com/2013/05/ama-says-ehrs-create-appalling-catch-22.html">noted</a>, even the AMA now admits that<br /><br /><blockquote class="tr_bq"> As the healthcare industry moves to EHRs, t<b>he medical record has essentially been reduced to a tool for billing, compliance, and litigation that also has a sustained negative impact on doctors' productivity,</b> according to Steven J. Stack, MD, chair of the American Medical Association’s board of trustees. </blockquote><br />Yet in this case, a well known commercial EHR did not even help out the hospital system's finances.<br /><br />Furthermore,<br /><br /><blockquote class="tr_bq">Wake Forest Baptist said it spent as of Dec. 31 about $13.3 million directly on the Epic electronic-record system, which went live in September.</blockquote><br />And,<br /><br /><blockquote class="tr_bq"> The center also cited $8 million in 'other Epic-related implementation expense' that it listed among '<i>business-cycle disruptions (that) have had a greater-than-anticipated impact on volumes and productivity</i>.' Also listed was $26.6 million in lost margin <i>'due to interim volume disruptions during initial go-live and post go-live optimization</i>.'</blockquote><br />Note that InformaticsMD frequently criticizes proponents of commercial health care information technology for glossing over potentially bad effects on patients and practice with management-speak (e.g., as "glitches," or "hiccups.").  Here is a great example of an attempt to gloss over bad effects on finance with management-speak. <br /><br /><b>Bond Downgrades and Furloughs, Wage Reductions, Hiring Freezes, Retirement Contribution Reductions, and Bonus Eliminations</b><br /><br />As a consequence,<br /><br /><blockquote class="tr_bq"> On March 20, <i>Moody’s Investors Service downgraded the center’s long-term debt rating below the lowest level of high-grade investment quality.</i> The downgrade to A1 from Aa3 affects $597.2 million of rated debt outstanding.</blockquote><br />The rationale was clear,<br /><br /><blockquote class="tr_bq"> Moody’s said the A1 rating 'reflects the unexpected decline in financial performance through the first half of fiscal 2013, largely due to the installation of a new information technology platform (Epic), encompassing 95 percent of all revenue components of the enterprise.'</blockquote>You know when you see bond downgrade by rating agencies that  financial matters are really going badly.   <br /><br />By May, 2013, month, the problems were evidently still not solved, and the hospital was forced to take more drastic measures.  As again <a href="http://www.journalnow.com/business/business_news/local/article_0cb3b3a8-b33d-11e2-a80f-0019bb30f31a.html">reported</a> by Richard Carver writing for the Winston-Salem Journal, <br /><br /><span class="paragraph-0"> </span><br /><blockquote class="tr_bq">The workforce at Wake Forest Baptist Medical Center is <i>paying a paycheck price to make up for the financial shortcomings to date of its Epic electronic records system</i>.<br /><br /><span class="paragraph-1"> The center said in a statement Thursday it has begun <i>another round of cost-cutting measures</i> that will last through at least June 30, the end of its 2012-13 fiscal year.<br /> </span> <br /><br />The measures include attempts at <i>volunteer employee furloughs and hour-and-wage reductions, a hiring freeze, a reduction in employer retirement contributions, and elimination of executive incentive bonuses for 2013.</i></blockquote><br /><br />Management made clear that the cuts were in response to the Epic debacle,<br /><br /><br /><blockquote class="tr_bq">Even though management said Thursday the center is making progress with fixing the Epic revenue issues, it acknowledged it 'will not meet projected financial targets for the current fiscal year.'<br /><br />'Wake Forest Baptist has identified immediate multimillion-dollar savings with a series of short-term measures that impact personnel,' according to the statement.</blockquote><br />To give credit where it is due, at least the cuts will apparently not affect line clinical employees:<br /><br /><blockquote class="tr_bq"> Those primarily affected by the volunteer furloughs and hour-and-wage reduction requests are nonclinical full-time employees, including administrative staff. They can volunteer to work as few as 30 hours a week with no loss of health or dental benefits for May and June. In the memo, management said employees can volunteer to continue the reduced-hour work week into fiscal year 2013-14.</blockquote><br />However, it seems likely that they will affect many employees, including some proportion who likely had not responsibility for the problems with Epic.<br /><br /><b>When in Doubt, Lobby the Government </b><br /><br />What the hospital system did not seem to be cutting was lobbying and public relations.  Perhaps this was a response to its unexpected inability to manage its own commercial health care information technology?  What bad management can lose, maybe government can supplant.  Once again Richard Carver had <a href="http://www.journalnow.com/news/state_region/article_1aee766e-b5e1-11e2-bba7-001a4bcf6878.html">the story</a> for the Journal:<br /><br /><span class="paragraph-0"> </span><br /><blockquote class="tr_bq">Stung by a series of unusual setbacks at the General Assembly, the North Carolina hospital industry is <i>launching a public relations campaign</i> aimed, in part, at protecting revenues and staving off competition from lower cost surgery centers.<br /><br /><span class="paragraph-1"> In a social media initiative targeted at lawmakers and their constituents, the N.C. Hospital Association says hospitals are 'fighting for their economic survival.' [It was not said whether they were fighting in part because they had already managed to shoot themselves in their economic feet - Ed]<br /><br /> </span> <br />The association and some of the state’s bigger hospitals also are hiring more GOP lobbyists to make inroads with the Republicans who control the state House, Senate and governor’s mansion.<br /><br />The hospital association recently began promoting a new website — <a href="http://www.healthyhospitalsnc.org/">www.healthyhospitalsnc.org</a> — that describes an array of financial threats.</blockquote><br />Wake Forest Baptist is a big part of this initiative:<br /><br /><blockquote class="tr_bq">When asked about its lobbying efforts, Wake Forest Baptist spokeswoman Paula Faria said last week that the center’s Office of Government Relations monitors proposed legislation and regulations at both the federal and state levels.<br /><br />'It informs North Carolina’s congressional delegation, members of the General Assembly and their staff about how proposed language could impact the day-to-day operations of the medical center.'</blockquote><br />Maybe they should be first worrying about the impact of badly chosen, designed, or implemented commercial health care information technology on "day-to-day operations of the medical center" first. <br /><br /><b> Summary</b><br /><br />So the top executives of Wake Forest Baptist Medical Center have seen compensation rising at a rate greater than inflation and than the general public's income over the last few years.  In particular, the CEO has seen his compensation go up three and one-half times in three years!  The hospital system administration has justified this extraordinary increase by referring to supposedly "visionary" leadership.  Yet over this time frame these "visionaries" decided to implement an EHR whose first effects were to lose the hospital system a lot of money.  Based on previous anecdotes about the Epic system, it is quite possible it had other adverse effects.  For example, InformaticsMD <a href="http://hcrenewal.blogspot.com/2012/09/in-addition-to-nurses-doctors-now-air.html">discussed</a> a case in which an EPIC system apparently lead to a large disruption in patient workflow and hence large increases in waits for acute care, and <a href="http://hcrenewal.blogspot.com/2012/08/contra-costas-45-million-computer.html">lead to errors</a> that could have adversely affected patients.  So this underscores some important lessons:<br /><br />So beware that "visionary" behind the curtain. As we have noted repeatedly, top health care managers can now easily make themselves rich.  They, their boards of directors (who may be their cronies), and their public relations flacks often justify their exorbitant compensation by their supposed brilliance, if not visionary status.  Such claims are rarely further explained, and mostly seem be be humbug, for want of a better term.  It seems that most top leaders of health care organizations have participated in the managers' coup d'etat, and become at least manager nobility, if not manager-kings  At least, the public should know that their compensation is what they can grab, and its justification is often nonsense. <br /><br />Note that contrary to a red herring argument often made, outrageous compensation is important not so much because of how much money it drains out of health care, although that can be large in the aggregate.  It is important because it reflects a system that is no longer accountable, and leaders who follow perverse incentives.<br /><br />Such management compensation is almost never revisited to determine whether it turned out to be justified.  Instead, the public, watchdog organizations, health care professionals, and even politicians ought to demand accountability of health care management, good  justification for their compensation, and rationality for the incentives they are provided.  True health care reform would encourage well-informed, competent, mission-focused, honest, responsible, accountable and transparent management, leading organizations of manageable size.  But as long as things stay the same, expect the craziness to continue.    <br /><br /><br />

Financial woes at Maine Medical Center: Reading this blog might have saved them millions of dollars, and prevented massive "cost saving initiatives"

In this article, the euphemistic and almost endearing term <b>"hiccup"</b> is used instead of the more traditional "<a href="http://hcrenewal.blogspot.com/search/label/glitch">glitch</a>" to describe obvious major information technology malfunctions.  It is likely the knowledge at this blog and at my <a href="http://www.ischool.drexel.edu/faculty/ssilverstein/cases/">health IT dysfunction teaching site</a> could have helped prevent most of these problems:<br /><br /><blockquote class="tr_bq"><b><a href="http://www.healthcarefinancenews.com/news/financial-woes-maine-medical?topic=03,22">Financial woes at Maine Medical Center</a></b><br />New England health system facing $13 million loss, initiates plan to save $15 million<br />NEW GLOUCESTER, ME | May 2, 2013 <br /><br />In a memo to its employees last week, one of Maine’s largest health systems said it has suffered an operating loss of $13.4 million in the first half of its fiscal year.<br /><br />“Through March (six months of our fiscal year), Maine Medical Center <b>experienced a negative financial position that it has not witnessed in recent memory</b>,” Richard Petersen, president and CEO of the medical center, wrote in the memo to employees. A copy of the memo was sent to MedTech Media, publisher of Healthcare Finance News.</blockquote><br /><b>A "negative financial position" </b>(translation: we lost big money)<b> that it has not witnessed in recent memory</b>?  What are the reasons?<br /><br /><blockquote class="tr_bq">In order to bring the medical center to breakeven by year’s end, the health system’s leadership has determined $15 million needs to be saved.<br /><br />In the memo, Petersen said the operating loss is due to declines in inpatient and outpatient volumes because of the hospital’s efforts to reduce readmissions and infections;<b> “unintended financial consequences” due to the roll out of the health system’s Epic electronic health record and problems associated with being unable to accurately charge for services provided</b>; an increase in free care and bad debt cases; and continued declining reimbursement from Medicare and MaineCare, the state’s Medicaid program.</blockquote><br />That rings a familiar tune - from the <a href="http://www.ischool.drexel.edu/faculty/ssilverstein/cases/?loc=cases&sloc=Insufficient%20IT%20management">mid 1990's</a> at Yale, as well as <a href="http://www.ischool.drexel.edu/faculty/ssilverstein/DUCOM_EMR_Complaint.pdf">more recently</a>.<br /><br /><blockquote class="tr_bq">Many of the reasons for Maine Medical’s financial woes are similar to those hospitals across the country are facing.<br /><br />A recovering national economy, continued budget restrictions and restraint and the realization that, while electronic health records may have efficiencies and cost savings over time, <b>the costly transition to EHRs may take years to recoup.</b></blockquote><br /><br />Especially when not done well.<br /><br /><blockquote class="tr_bq">In his memo to employees, Petersen said the hospital has <b>identified many of the hiccups contributing to the charge capture problems</b> and a team of hospital employees and Epic technicians are working to resolve those issues. In the meantime, the <b>remaining roll out of the Epic EHR to the rest of the health system is on hold.</b></blockquote><br />Hiccups? Health IT has a euphemistic language all its own.  Only apostates would dare to call the "hiccups" for what they really are, in medical parlance:  <b>IT malpractice.</b><br /><blockquote class="tr_bq"><br />To save $15 million by year’s end, Maine Medical is immediately instituting a number of cost-saving initiatives including selective travel and <b>hiring freezes</b>, putting the <b>operating contingency budget on hold</b> and <b>reducing overtime. </b>Petersen appealed to employees to curb discretionary spending and contact management with any cost-saving ideas.</blockquote><br />All, of course, will have no impact on patient care....<br /><blockquote class="tr_bq"><br />“I’m confident that we’ll confront this <b>test,</b> beat back the issues we face, and reverse this negative financial picture,” Petersen wrote in the conclusion to the memo.</blockquote><br />Test?  Test of what, IT competence?<br /><br />Of course, "C" officers would never write that "I'm <b>not </b>confident we'll confront our <b>screwups</b>."<br /><br /><blockquote class="tr_bq">Maine Medical did not reply to an interview request by deadline. This has been an unprecedented cooperation between 4 towns in this region that has generated many continuing activities that will benefit all towns in our region.</blockquote><br />Apparently an entire hospital is closing as a result of these debacles.<br /><br /><i><b>[May 8, 2013 addendum: a family physician in Lincoln County where St. Andrews hospital is located and a member of the Board of Trustees, </b></i><i><b>Dan Friedland, M.D., writes me that the EHR had nothing to do with the hospital closing - ed.] </b></i><br /><br /><blockquote class="tr_bq">But let me get back to the MH/LCH decision. <b>We are told that MaineHealth has spent over $150 million on an Electronic Medical Records (EMR)</b> system that helps all of its “subsidiaries.” I can appreciate this because my work is in software development.<br /><br /><b>I do question the $150 million figure. I think it is extremely high and Portland has had a real failure in its implementation. </b>So much so that it looks like LCH will not have a real integrated EMR until 2015 and financial software problems exemplify a major failure of MH to create any real benefit to the State. Millions of dollars have been charged to member hospitals and staff time (salaries and mileage) over the past 2-3 years with no benefit.</blockquote><br />I'd questioned the high cost of these commercial EHR systems as far back as 2006 ("<a href="http://msnbc.msn.com/id/11693923/">Yet another clinical IT controversy: UC Davis</a>" and "<a href="http://hcrenewal.blogspot.com/2007/06/oversight-needed-for-hospital-emr.html">External oversight needed for hospital EMR implementation?</a>" - Lancaster General Hospital and "<a href="http://hcrenewal.blogspot.com/2006/10/70-million-for-electronic-medical.html">$70 million for an Electronic Medical Records system [quasi endpoint]?</a>- Geisinger).<br /><br />One might think healthcare systems have money to burn ...<br /><blockquote class="tr_bq"><br />The system failure also <b>adds operational costs going forward that were not planned for</b> and regional consolidation of finance will now be delayed. The cost to Maine Health Center is huge in improper service and supply charges. <b>Information Technology leadership has been fired</b>, but MH administration is truly accountable.</blockquote><br />For once, someone in IT leadership did not get a a <a href="http://www.ischool.drexel.edu/faculty/ssilverstein/cases/?loc=cases&sloc=Cardiology%20story">promotion for failure</a>.  It is true that MH administration is accountable, however - they had the fiduciary responsibility to hire the best talent, and to oversee that talent as needed to assure success.  If "C" leadership didn't understand IT, that's their failure as well.  In my view in 2013 everyone in a position of organizational responsibility should have a good understanding of IT, which is now, after all, a commodity.<br /><br />I'm hopeful EPIC, with its apparently <a href="http://hcrenewal.blogspot.com/2010/08/epics-outrageous-recommendations-on.html">revolutionary hiring practices</a> akin to the hiring of physicians, will have the "hiccups" fixed in no time.  From <a href="http://histalk2.com/2010/08/14/monday-morning-update-81610/">this link</a> at the "Histalk" site on staffing of health IT projects, Aug. 16, 2010. Emphases mine:<br /><br /><blockquote class="tr_bq">Epic Staffing Guide<br /><br />A reader sent over a copy of the staffing guide that Epic provides to its customers. I thought it was interesting, first and foremost in that Epic is so specific in its implementation plan that it sends customers an 18-page document on how staff their part of the project.<br /><br />Epic emphasizes that many hospitals can staff their projects internally, choosing people who know the organization. However, they emphasize choosing the best and brightest, not those with time to spare. <b>Epic advocates the same approach it takes in its own hiring: don’t worry about relevant experience, choose people with the right traits, qualities, and skills, they say.</b>  The guide <b>suggests hiring recent college graduates for analyst roles</b>. Ability is <b>more important than experience,</b> it says. The HIT Scam

John Hoyt, the Executive Vice President of the Healthcare Information and Management Systems Society (HIMSS) was quoted in a recent Health Change Bulletin as saying −<br /><br /><div style="padding-left: 30px;">This data suggests that the HITECH portion of the 2009 stimulus law is achieving its intended result of encouraging increased implementation and meaningful use of electronic health records among hospitals. Facilities…are laying the groundwork for interoperability to occur. Stage 6 and Stage 7 hospitals are fully prepared for provider-to-provider or facility-to-facility interoperability, as well as increasing the provider or facility’s ability to provide electronic health data reporting to public health and immunization registries to support population health review and syndromic surveillance.</div><div style="padding-left: 30px;"><br /></div>There, aren’t you greatly reassured? By the way, the <i>New York Times</i> piece cited above reported that –<br /><br /><div style="padding-left: 30px;">RAND’s <a href="http://www.rand.org/pubs/monographs/MG408.html" title="Abstract of the report.">2005 report</a> was paid for by a group of companies, including General Electric and Cerner Corporation, that have profited by developing and selling electronic records systems to hospitals and physician practices. Cerner’s revenue has nearly tripled since the report was released, to a projected $3 billion in 2013, from $1 billion in 2005.</div><div style="padding-left: 30px;"><br /></div>No doubt the companies that paid for the RAND study are also members of HIMSS. House Ways And Means, and Energy and Commerce, Note EHRs Not What They Were Made Out To Be, Calls For HITECH Moratorium

I have called numerous times for a moratorium on ambitious national health IT programs.  See 2008 and 2009 posts <a href="http://hcrenewal.blogspot.com/2008/11/should-us-call-moratorium-on-ambitious.html" target="_blank">here</a> and <a href="http://hcrenewal.blogspot.com/2009/01/i-ask-again-should-us-call-moratorium.html" target="_blank">here</a> for example.  My calls are due to the prevalence of bad health IT (BHIT) in 2012, hopelessly <a href="http://hcrenewal.blogspot.com/2010/08/epics-outrageous-recommendations-on.html" target="_blank">deficient</a> if not <a href="http://www.ischool.drexel.edu/faculty/ssilverstein/cases/?loc=cases&sloc=leadership" target="_blank">deranged</a> talent management practices (especially when compared to clinical medicine) in the health IT industry, and complete lack of regulation, validation and quality control of these potentially harmful medical devices.  <br /><br />I also called the HITECH stimulus act in its present form social policy malpractice.  (See my Sept. 2012 post "<a href="http://hcrenewal.blogspot.com/2012/09/at-risk-in-computerized-hospital-hitech.html" target="_blank">At Risk in the Computerized Hospital: The HITECH Act as Social Policy Malpractice, and Passivity of Medical Professional"</a>.) <br /><br />Congress is starting to catch on:<br /><br /><br /><table align="center" cellpadding="0" cellspacing="0" class="tr-caption-container" style="margin-left: auto; margin-right: auto;"><img border="0" height="320" src="https://blogger.googleusercontent.com/img/b/R29vZ2xl/AVvXsEhLUtBiwlwYFH7ZvyHvCqX8GOF5-2RDF_Zqeup_EImejMBCJrj2QbFDMoflwHj4vx0Kvx8IWRZXoFoBUX10_FeuYLKunSOdWWbiOU1IVsno1jC0OlcHJiUwZ8hkNcsq3PUyTsR3KwRWzIHs/s320/congress.bmp" width="252" /></a></td></tr><tr><td class="tr-caption" style="text-align: center;">Letter from House Ways and Means, and Energy & Commerce, to Secretary Sebelius of HHS.  Click <a href="http://waysandmeans.house.gov/uploadedfiles/hhs_ehr_mu2_final.pdf" target="_blank">here</a> to download.</td></tr></tbody></table><br />The letter to HHS secretary Sebelius is from Congressmen Dave Camp (Chairman, Ways and Means), Wally Herger (Chariman, Ways and Means Subcommittee on Health), Fred Upton (Chairman, Energy and Commerce) and Joe Pitts (Chairman, Energy and Commerce Subcommittee on Health).<br /><br />In the letter the following is noted:<br /><br /><blockquote class="tr_bq">Dear Secretary Sebelius: <br /><br />We are writing to express serious concerns about the final Electronic Health Record (EHR) Stage 2 Meaningful Use rules recently issued by HHS and ONC.  We believe the Stage 2 rules are, in some respects, weaker than the proposed Stage 1 regulation released in 2009.  The results will be a less efficient system <b>that squanders taxpayer dollars and does little, if anything, to improve outcomes for Medicare</b>.</blockquote><br />The letter then notes that the "Stage 2 rules ask less of providers and do less for program efficiency" and that the Stage 2 rules fail to achieve comprehensive interoperability in the face of<br />warnings that:<br /><br /><blockquote class="tr_bq">..".failure to set a date for certain interoperable standards would put as much as $35 billion in Medicare and taxpayer funds in the hands of providrrs who purchase and use EHR systems that are not interoperable."</blockquote><br />They note the Stage 2 rules fail to achieve interoperability in a timely manner and that "more than four and a half years and two final MU rules later, it is safe to say that we are no closer to interoperability in spite of the nearly $10 billion spent."<br /><br />A major reason for this, I believe, is <b>regulatory capture</b> by the IT industry as I outlined in my somewhat rhetorically-entitled posts "<a href="http://hcrenewal.blogspot.com/2012/08/health-it-vendor-epic-uses-clients-as.html" target="_blank">Health IT Vendor EPIC Caught Red-Handed: Ghostwriting And Using Customers as Stealth Lobbyists - Did ONC Ignore This?</a>" and "<a href="http://hcrenewal.blogspot.com/2012/09/from-what-i-can-tell-epic-was.html" target="_blank">Was EPIC successful in watering down the Meaningful Use Stage 2 Final Rule?</a>"<br /><br />The House letter also notes:<br /><br /><blockquote class="tr_bq">It is highly counterproductive for providers to have purchased EHR systems that "cannot talk with one another" and <b>cannot perform basic functions </b>because of the insufficient standards set by your agency. </blockquote><br />One of the critical "basic functions" is the note search capability upon which the vendors used their influence during the "public comments" period to have written out of existence, as in the above posts.  The influence became apparent due to serious public comment editing mistakes by customers.  One wonders what other episodes of vendor influence did not make it into the public spotlight.<br /><br />The house committee members also note:<br /><br /><blockquote class="tr_bq">Perhaps not surprisingly, your EHR inventive program appears to be doing more harm than good.  A recent analysis of Medicare data by the New York Times explains the costly consequences.</blockquote><br />Unfortunately, the letter did not spotlight the excellent analysis done by the Center for Public Integrity and published before the NYT article ("<a href="http://www.publicintegrity.org/2012/09/15/10810/how-doctors-and-hospitals-have-collected-billions-questionable-medicare-fees" target="_blank">Cracking the Codes</a>" by Fred Schulte et al.)<br /><br />Finally, the letter calls for HHS to:<br /><br /><blockquote class="tr_bq">... Immediately suspend the distribution of incentive payments until your agency promulgates universal interoperable standards.  Such a move would also require a commensurate delay of penalties for providers who choose not to integrate HIT into their practice"  and to "significantly increase what's expected of Meaningful Users." </blockquote><br /><br />It is unfortunate the letter seems to make the assumption that health IT in its present form, and the industry in its present state of anarchy,<b> can produce good health IT (GHIT) that is safe</b> <b>and effective. </b> (As I've written, we need ease-of-use, reliability and safety - basic "operability" - before interoperability.)  Perhaps the congresspeople need to read my recent post "<a href="http://hcrenewal.blogspot.com/2012/10/honesty-and-good-sense-on-electronic.html" target="_blank">Honesty and Good Sense on Electronic Medical Records From Down Under</a>".<br /><br />Financial issues are one major concern, but<b> patient harm and death due to the disruptive influences of BHIT</b> are, in fact in many respects more important.<br /><br />Finally, to those who would suggest a political angle to this letter (I note comments on sites such as on the <a href="http://histalk2.com/2012/10/04/news-10512/" target="_blank">Histalk</a> blog that the authors are Republicans), I note that ONC was started in 2004 by George W. Bush, and that health IT has always had broad bi-partisan support.<br /><br />Reality in healthcare is more often than not apolitical, and injured and dead patients really don't care much about ideology.<br /><br />-- SS

WSJ, Koppel and Soumerai: A Major Glitch for Digitized Health-Care Records - Cost Savings Illusory

Two EHR experts from Ivy-league institutions (which are typically hyperenthusiast-oriented) doubt cost savings from EHR technology.<br /><br /><blockquote class="tr_bq"><b>Wall Street Journal<br />Opinion<br />September 17, 2012<br /><br /><a href="http://online.wsj.com/article/SB10000872396390443847404577627041964831020.html?mod=googlenews_wsj" target="_blank">A Major Glitch for Digitized Health-Care Records</a></b><br /><br />Savings promised by the government and vendors of information technology are little more than hype.<br /><br />By <a href="http://www.populationmedicine.org/content/personnelDetail.asp?PID=164&CID=1&Sub=Y" target="_blank">STEPHEN SOUMERAI</a> And <a href="http://sociology.sas.upenn.edu/r_koppel" target="_blank">ROSS KOPPEL</a><br /><br />In two years, hundreds of thousands of American physicians and thousands of hospitals that fail to buy and install costly health-care information technologies—such as digital records for prescriptions and patient histories—will face penalties through reduced Medicare and Medicaid payments. At the same time, the government expects to pay out tens of billions of dollars in subsidies and incentives to providers who install these technology programs. </blockquote><br />Which, of course, means printing more money for luxuries we cannot afford in the midst of the worst economic downturn since 1929...<br /><br /><blockquote class="tr_bq">The mandate, part of the 2009 stimulus legislation <span style="color: #cc0000;"><i><b>[I think it would be more accurate to say "sneaked into the 2009 stimulus legislation for IT industry benefit" - ed.]</b></i></span>, was a major goal of health-care information technology lobbyists and their allies in Congress and the White House. The lobbyists promised that these technologies would make medical administration more efficient and lower medical costs <b>by up to $100 billion annually. </b></blockquote><br /><b>By up to $100 billion annually?   </b>(Perhaps these lobbyists need lie detector tests - and/or drug tests to see what they may have been smoking?)  I note the lobbying was covered in the Washington Post in "<span style="font-size: 100%;"><a href="http://www.washingtonpost.com/wp-dyn/content/article/2009/05/15/AR2009051503667.html?hpid=topnews">The Machinery Behind Health-Care Reform: How an Industry Lobby Scored a Swift, Unexpected Victory by Channeling Billions to Electronic Records</a>" by Robert O'Harrow Jr., May 16, 2009</span>.<br /><br /><blockquote class="tr_bq">Many doctors and health-care administrators are wary of such claims—a wariness based on their own experience. An extensive new study indicates that the caution is justified: The savings turn out to be chimerical.</blockquote><br />Some still have empirical observational skills and common sense, fortunately.  In my early days of health IT involvement two decades ago as an NIH-sponsored Medical Informatics post doctoral fellow, I believed there was some cost savings possible, but certainly not in the tens or hundreds of billions per year.  The technology was not solving problems that could lead to such savings (see for instance my Dec. 2010 post "<a href="http://hcrenewal.blogspot.com/2010/12/is-healthcare-it-solution-to-wrong.html" target="_blank">Is Healthcare IT a Solution to the Wrong Problem?</a>").  My experiences since my fellowship have only confirmed my own early opinions.<br /><br /><blockquote class="tr_bq">Since 2009, almost a third of health providers, a group that ranges from small private practices to huge hospitals—have installed at least some "health IT" technology. It wasn't cheap. For a major hospital, a full suite of technology products can cost $150 million to $200 million. Implementation—linking and integrating systems, training, data entry and the like—can raise the total bill to $1 billion.</blockquote><br />That's countless billions that could have been spent on actual healthcare delivery itself, especially for underserved populations and the poor.  Instead, that money was funnelled to the IT industry for an experimental, unregulated, unvetted technology to be applied on live patients (and without patient consent, I add).<br /><br /><blockquote class="tr_bq">But the software—sold by hundreds of health IT firms—<b>is generally clunky, frustrating, user-unfriendly and inefficient.</b> For instance, a doctor looking for a patient's current medications might have to click and scroll through many different screens to find that essential information. Depending on where and when information on a patient's prescriptions were entered, the complete list of medications may only be found across five different screens.</blockquote><br />Examples of that from actual systems (sketched to avoid IP problems) is at my ten part series on <b>mission-hostile health IT</b> at this blog, shortened link <a href="http://www.tinyurl.com/hostileuserexper">http://www.tinyurl.com/hostileuserexper</a>.<br /><br /><blockquote class="tr_bq">Now, a comprehensive evaluation of the scientific literature has confirmed what many researchers suspected: The savings claimed by government agencies and vendors of health IT <b>are little more than hype.</b><br /><br />To conduct the study, faculty at McMaster University in Hamilton, Ontario, and its programs for assessment of technology in health—and other research centers, including in the U.S.—sifted through almost 36,000 studies of health IT. The studies included information about highly valued computerized alerts—when drugs are prescribed, for instance—to prevent drug interactions and dosage errors. From among those studies the researchers identified 31 that specifically examined the outcomes in light of the technology's cost-savings claims.<br /><br />With a few isolated exceptions, the preponderance of evidence shows that the systems had not improved health or saved money. </blockquote><br />One must understand this in the context of what these systems really are, what they do, <b>and the problems they can actually address</b> as in my post on 'solution to the wrong problem' mentioned <u>supra</u>.<br /><br /><blockquote class="tr_bq">The authors of "The Economics of Health Information Technology in Medication Management: A Systematic Review of Economic Evaluations" <b>found no evidence from four to five decades of studies that health IT reduces overall health costs.</b> Three studies examined in that McMaster review incorporated the gold standard of evidence: <b>large randomized, controlled trials. </b>They provide the best measure of the effects of health IT systems on total medical costs.</blockquote><br />I point out that<i> randomized controlled trials (RCT's) are notably rare in health IT studies.</i>  Most "confirmatory" studies on health IT benefits are less powerful retrospective/observational studies in specialized settings, with many biases, poor science, etc.  In that sense, such studies can be considered non-generalizable and <b>anecdotal</b> at best (more on the "anecdotes" issue is below).<br /><br />Of course, the pundits and Ddulites ("Luddite" with first four letters reversed; meaning ardent hyperenthusiasts) will likely say "<i>it will all be fixed in version 2.0</i>" despite decades of opportunity to get health IT "right" and demonstrate generalized improved outcomes and cost savings:<br /><blockquote class="tr_bq"><br />... At "<a href="http://hcrenewal.blogspot.com/2012/02/health-it-ddulites-and-irrational.html">Health IT: Ddulites and Irrational Exuberance</a>" and related posts (<a href="http://hcrenewal.blogspot.com/search/label/Ddulite">query link</a>) I've described the phenomenon of the:<span style="font-style: italic;"></span><span style="color: red; font-style: italic; font-weight: bold;"> 'Hyper-enthusiastic technophile who either deliberately ignores or is blinded to technology's downsides, ethical issues, and repeated local and mass failures.</span><span style="color: red; font-weight: bold;">'</span></blockquote><br />From Regenstrief Institute, one of the premier and pioneering centers for Medical Informatics research and teaching:<br /><br /><blockquote class="tr_bq">A study from Regenstrief, a leading health IT research center associated with the Indiana University School of Medicine, found that there were no savings, and another from the same center found a significant increase in costs of $2,200 per doctor per year. The third study measured a small and statistically questionable savings of $22 per patient each year.</blockquote><br />That's probably not taking into account <b>increased medical malpractice costs </b>as exemplified <a href="http://hcrenewal.blogspot.com/2011/06/babys-death-spotlights-safety-risks.html" target="_blank">here</a> and <a href="http://hcrenewal.blogspot.com/2011/09/sweet-death-that-wasnt-very-sweet-how_24.html" target="_blank">here</a> and by other settlements I know of but cannot mention due to my consulting role in those cases.<br /><br /><b>A primer on health IT risks,</b> containing a number of key articles and links, is downloadable at <a href="http://www.ischool.drexel.edu/faculty/ssilverstein/HIT_issues_Primer.zip">http://www.ischool.drexel.edu/faculty/ssilverstein/HIT_issues_Primer.zip</a>.  <br /><br /><blockquote class="tr_bq">In short, the most rigorous studies to date contradict the widely broadcast claims that the national investment in health IT—some $1 trillion will be spent, by our estimate—will pay off in reducing medical costs. Those studies that do claim savings rarely include the full cost of installation, training and maintenance—a large chunk of that trillion dollars—for the nation's nearly 6,000 hospitals and more than 600,000 physicians.</blockquote><br />$1 trillion would pay for a tremendous amount of actual healthcare delivery, I note.<br /><br />These system lifecycle costs, known as "total cost of ownership", are due to upgrades and necessary changes as medicine itself, including the science, business and legal components change over time.   The costs are generally underestimated ahead of time.  A good source on these issues is the article "<a href="http://www.ischool.drexel.edu/faculty/ssilverstein/Goldfinch_pessimism_PAR.pdf" target="_blank">Pessimism, Computer Failure, and Information Systems Development in the Public Sector</a>"  (Public Administration Review 67;5:917-929, Sept/Oct. 2007).  This is a cautionary article on IT that recommends much more critical attitudes towards major IT initiatives in all sectors.  <br /><br /><blockquote class="tr_bq">But by the time these health-care providers find out that the promised cost savings are an illusion, it will be too late. Having spent hundreds of millions on the technology, they won't be able to afford to throw it out like a defective toaster.</blockquote><br />There, I disagree.  We may have to "throw it out" as the disruptions to good medical care by bad health IT (BHIT) increase,<b> unless the industry reforms itself very rapidly</b>.  As I state in the introduction to my teaching website "<a href="http://www.ischool.drexel.edu/faculty/ssilverstein/cases/" target="_blank">Contemporary Issues in Medical Informatics: Good IT, Bad IT, and Common Examples of Healthcare Information Technology Difficulties</a>" (started in late 1998 due, in fact, to my observations of inappropriate leadership and poor quality of health IT):<br /><br /><blockquote class="tr_bq"><span style="font-family: Arial;">The site takes a view consistent with medicine’s core values and with patients’ rights that health IT, as a medical apparatus that increasingly forms an enterprise clinical resource management and clinician workflow control mechanism, remains experimental and should be subject to the same ethical considerations, validation processes and regulatory oversight to which almost all other medical technologies and pharmaceuticals are subjected. <b>The site is pro-health IT</b>, but <i><b>only when the health IT is “good IT” - as opposed to “bad IT."</b></i> <br /><b><br />Good Health IT ("GHIT")</b> is defined as IT that provides a good user experience, enhances cognitive function, puts essential information as effortlessly as possible into the physician’s hands, keeps eHealth information secure, protects patient privacy and facilitates better practice of medicine and better outcomes. <br /><b><br />Bad Health IT ("BHIT") </b>is defined as IT that is ill-suited to purpose, hard to use, unreliable, loses data or provides incorrect data, causes cognitive overload, slows rather than facilitates users, lacks appropriate alerts, creates the need for hypervigilance (i.e., towards avoiding IT-related mishaps) that increases stress, is lacking in security, </span><span style="font-family: Arial;"><span style="font-size: small;"><span style="font-family: inherit;"><span style="color: black;"><span style="color: black; font-family: Arial; mso-bidi-font-weight: bold;"><span style="color: black; font-family: Arial; mso-bidi-font-weight: bold;">compromises</span></span></span></span></span> patient privacy or otherwise demonstrates suboptimal design and/or implementation.</span><br /><br /><span style="font-family: Arial;">This site challenges medically and ethically controversial views that health IT merits special accommodations in terms of freedom from scientific rigor and evidence-based practices, freedom from regulation, and freedom from accountability.  </span></blockquote><br />I  hold little hope of these issues being seriously addressed - let alone solved - any time in the near future due to:<br /><br />1)  the irrational exuberance about this technology, cf: Twitter feeds on hashtags #EHR, #EMR, #HCIT and similar; <br />2)  the industry-promoted myths and fabrications over decades;<br />3)  the FDA-admitted impediments to information diffusion about defects and harms (<a href="http://hcrenewal.blogspot.com/2010/08/smoking-gun-internal-fda-memorandum-of.html" target="_blank">link</a>); <br />4)  IOM-admitted impediments to same (<a href="http://hcrenewal.blogspot.com/2011/11/iom-report-on-health-it-safety-nix-fda.html" target="_blank">link</a>);and<br />5)  the latter's industry-favoring, Milquetoast 2012 recommendations on monitoring, remediation and regulation.<br /><br />These are indicative of an industry that has neatly avoided any meaningful regulation and is, in plain terms, out of control.<br /><br /><blockquote class="tr_bq">Why are we pushing ahead to digitize even more of the health-care system, when the technology record so far is so disappointing? <b>So strong is the belief in health IT that skeptics and their science are not always welcome.</b> Studies published several years ago in the Journal of the American Medical Association and the Annals of Internal Medicine reported that health IT systems evaluated by their own developers were far more likely to be judged "successful" than those assessed by independent evaluators.<br /><br /></blockquote>"Not welcome" is an <i>understatement. </i><br /><br />ONC itself is guilty of bad science, as in its grossly deficient March 2011 Health Affairs paper on health IT benefits. (See my Oct. 2011 post "<a href="http://hcrenewal.blogspot.com/2011/03/benefits-of-health-information.html" target="_blank">ONC: The Benefits Of Health Information Technology: A Review Of The Recent Literature Shows Predominantly Positive Results</a>".  Also see my Aug. 2012 post "<a href="http://hcrenewal.blogspot.com/2012/08/onc-and-irrelevant-comparisons.html" target="_blank">ONC and Misdirection Regarding Mass Healthcare IT Failure</a>".)<br /><br />So bad is the <b>"Health IT critique not welcome"</b> phenomemon that, for example, Medical Informatics leaders such as Bill Hersh of Oregon Health Sciences University (OHSU) openly attack those who are candid on health IT risks (and make fools of themselves via the open attacks), such as the attack I wrote of at my Sept. 2010 post "<a href="http://hcrenewal.blogspot.com/2010/09/dangers-of-critical-thinking-in.html" target="_blank">The Dangers of Critical Thinking in A Politicized, Irrational Culture</a>".<br /><br />Also common in the "Health IT Critique Not Welcome Here" club, and probably deliberately so, is mistaking risk management activities for research observations and calling the former, even when reported by credible witnesses, <b>"anecdotal"</b> while calling at-best weak HIT-positive studies <b>"definitive science."</b>  See "<a href="http://hcrenewal.blogspot.com/2011/08/from-senior-clinician-down-under.html" target="_blank">From a Senior Clinician Down Under: Anecdotes and Medicine, We are Actually Talking About Two Different Things</a>" for the definitive rebuttal of that type of patient-endangering intellectual blindness or dishonesty.<br /><br /><blockquote class="tr_bq">Government agencies like the Office of the National Coordinator of Healthcare Information Technology (an agency of the Department of Health and Human Services) serve as health IT industry boosters. ONC routinely <b>touts stories of the technology's alleged benefits.</b></blockquote><br />But almost never the opposite, which is equally if not more important.  Further, they tout "certification" as a means of HIT safety evaluation and assurance, or do not deny that misconception, despite statements by the very certification bodies or ATCB's they appoint to the contrary.  Yet they need to come clean on this. I have similar problems with the way President Obama hopes to pay for the huge and costly health reform package he has in mind that will cover all Americans; he is counting on the “savings” that will come as a result of <b>investing in preventive care and investing in the electronic medical record among other things. It’s a dangerous and probably an incorrect projection. </b></blockquote><br />I also had related observations published in a Letter to the Editor in the WSJ in February of that year:<br /><br /><blockquote class="tr_bq"><a href="http://online.wsj.com/article/SB123492035330205101.html" target="_blank">Digitizing Medical Records May Help, but It's Complex</a>, Feb. 18, 2009<br /><br />You observe that the true political goal is socialized medicine facilitated by health care information technology. You note that the public is being deceived, as the rules behind this takeover were stealthily inserted in the stimulus bill.<br /><br />I have a different view on who is deceiving whom. In fact, it is the government that has been deceived by the HIT industry and its pundits. Stated directly, the administration is deluded about the true difficulty of making large-scale health IT work. The beneficiaries will largely be the IT industry and IT management consultants.<br /><br />For £12.7 billion the U.K., which already has socialized medicine, still does not have a working national HIT system, but instead has a major IT quagmire, some of it caused by U.S. HIT vendors.<br /><br />HIT (with a few exceptions) is largely a disaster. I'm far more concerned about a mega-expensive IT misadventure than an IT-empowered takeover of medicine.<br /><br />The stimulus bill, to its credit, recognizes the need for research on improving HIT. Vermont: Despite $70 million investment, health IT systems a long way from prime time - "Problems are appropriate"

<div style="color: red; text-align: center;"><span style="font-size: small;"><b><i>Preliminary note:  This post is rich with hyperlinks.  At minimum I recommend opening them in another window and at least scanning their contents - SS</i></b></span></div><br />No surprises in this article, including an amorality alien to the healing professions, but common in technology circles:<br /><blockquote class="tr_bq"><br /><span style="font-size: small;"><b><a href="http://vtdigger.org/2012/07/18/despite-70-million-investment-health-it-systems-a-long-way-from-prime-time/" target="_blank">Despite $70 million investment, health IT systems a long way from prime time</a></b></span><br />VTDigger.org<br />Andrew Nemethy<br />July 18, 2012<br /><br />The state’s efforts to digitize the world of health information, a costly multi-year endeavor that is <b>approaching a $70 million pricetag,</b> got a lousy diagnosis Tuesday.<br /><br />Instead of creating cost efficiency and improving payment flow to doctors and treatment for patients, it’s <b>creating stress and a lot of headaches for physicians</b>, according to both lawmakers and state officials overseeing the effort. <i style="color: red;"><b>[It's also creating <u>increased risk</u> for patients, a factor - the most crucial factor - rarely mentioned in articles such as this - ed.]</b></i></blockquote><br />Money, of course, grows on trees, and physicians, hospitals and government have nothing better to do with $70 million than conduct experiments on patients with alpha and beta software ... <br /><br />There's the usual excuses from the usual actors: <br /><br /><blockquote class="tr_bq">But Health Information Technology (HIT) coordinator Hunt Blair said that’s to be expected considering the difficulty of the “incredibly challenging” task of getting such disparate groups as doctors, hospitals, other health care providers, insurance companies, the state and federal government on the same digital page. </blockquote><br />Let alone (per <a href="http://www.dlib.org/dlib/january99/kling/01kling.html" target="_blank">Social Informatics</a>) the organizational and sociological challenges of implementing any new information or communications technology (ICT), that's somewhat akin to saying it's hard to get people to consume arsenic as an aphrodisiac.  Not mentioned is the deplorable state of health IT in terms of quality, safety, usability, unregulated nature etc.<br /><br /><blockquote class="tr_bq">“We’re talking about an extremely complex undertaking and I think it’s important to recognize the state of Vermont was way out in front,” Blair said.<br /><br /><b>“We’re on the bleeding edge,”</b> he told a legislative Health Care Oversight Committee Tuesday at the Statehouse. <br /><br />That prompted Sen. Claire Ayer, D-Addison, the panel’s chairwoman, to ask him to clarify if he meant “leading.” <br /><br />He stuck with <b>“bleeding.”</b></blockquote><br />"Bleeding edge?"<br /><br />Aside from the very poor choice of terms, this attitude is the polar opposite of the culture of "first, do no harm."  It is not a clinician's attitude.  It is an attitude of someone who seems to forget that patients are at the receiving end of the "bleeding edge" (which usually implies a rocky course) technology:<br /><br /><blockquote class="tr_bq"><a href="http://en.wikipedia.org/wiki/Bleeding_edge_technology" target="_blank"><b>Bleeding edge technology</b></a> is a category of technologies incorporating those so new that they could have a <b>high risk of being unreliable </b>and lead adopters to incur greater expense in order to make use of them.  The term bleeding edge was formed as an allusion to the similar terms "leading edge" and "cutting edge". It tends to imply even greater advancement, albeit at an <b>increased risk of "metaphorically cutting until bleeding" because of the unreliability of the software or other technology.</b>The phrase was originally coined in an article entitled "Rumors of the Future and the Digital Circus" by Jack Dale, published in Editor & Publisher Magazine, February 12, 1994.<a href="http://en.wikipedia.org/wiki/Bleeding_edge_technology#cite_note-0"><span></span></a></blockquote><br />Wonderful.<br /><br />Considering the risks to patients, this claim brings to mind the definition I posted of the <a href="http://hcrenewal.blogspot.com/2012/03/doctors-and-ehrs-reframing-modernists-v.html" target="_blank"><b>health IT Ddulite</b></a> ("Luddite", the common canard against cautious doctors, with the first four letters reversed):<br /><br /><blockquote><span style="font-weight: bold;"><span style="color: red; font-style: italic;">Ddulite:  Hyper-enthusiastic technophiles who </span></span><span style="color: red; font-style: italic; font-weight: bold;">either deliberately ignore or are blinded to technology's downsides</span><span style="font-weight: bold;"><span style="color: red; font-style: italic;">, ethical issues, and repeated local and mass failures. </span></span></blockquote><br />On the other hand, did critic VT Sen. Kevin Mullin read my <a href="http://www.ischool.drexel.edu/faculty/ssilverstein/cases/" target="_blank">Drexel website on health IT failure and mismanagement</a>?<br /><br /><blockquote class="tr_bq">That doesn’t surprise <b>Sen. Kevin Mullin, R-Rutland, who had tough questions</b> about the state’s effort to oversee and promote use of electronic medical health records and a statewide health information exchange.<br /><br />“I hear genuine frustration from providers who are spending time and resources trying to modernize and make their offices more efficient, and prepare for the future, <b>and yet every one of them feels like they’ve been burned,”</b> he said.<br /><br />“Basically <b>we’re not getting any results for these millions and millions of dollars that have been pumped into IT (information technology),</b>” he said after the meeting.<br /><br />“We should be a lot further along,” he said. <b>“I just don’t think the leadership’s in place.</b>”</blockquote><br />He's on the right track, but I'm not sure Vermont (or any state government) really understands what levels of leadership are truly needed, e.g., as outlined by ONC a few years ago <a href="http://hcrenewal.blogspot.com/2009/12/onc-defines-taxonomy-of-health-it.html" target="_blank">here</a>.<br /><br />More excuses:<br /><br /><blockquote class="tr_bq">... Mark Larson, commissioner of the Vermont Health Access Department and a former House representative from Burlington, oversees management of Vermont’s publicly funded health insurance programs and the effort on digital medical records and a new medical information exchange.<br /><br />... Larson told lawmakers he hears the same message they do, that there’s “a lot of confusion in the field.” He said that is an inevitable part of the complex process.<br /><br />“These are not systems where you just plug that in and they work perfectly on day one,” he said. <b>“Problems are appropriate along the path to get where we want."</b> <i style="color: red;"><b>[I note this is an implicit admission that experimentation is being performed - ed.]</b></i><b><br /></b><br /><br /><b>“We just have to work through that,” he said.<i> </i></b><b><br /></b></blockquote><br /><b> </b><br /><b>"Problems are appropriate?"  </b>... Really?<b>  In a mission critical field such as medicine?  </b>That's a maddeningly reckless and cavalier ideology, to put it mildly.<b>   </b>In what other safety-critical domain would such a happy-go-lucky attitude that "problems are appropriate", outside of the laboratory, be tolerated?  <br /><br /><b>"We just have to work through that?" </b>... "Just work through that?"  Really?<br /><br />That should be really easy,  just like the failed £12.7bn (~ US $20 billion) National Programme for IT (NPfIT) in the NHS, a <a href="http://hcrenewal.blogspot.com/2011/09/npfit-programme-going-pffft.html" target="_blank">program that went "Pffft"</a> last year.<br /><br /><blockquote class="tr_bq">... Based on testimony Tuesday, the issues that medical practitioners and the industry face in digitizing information are <b>familiar ones for anyone who deals with technology</b>: Software that is problematic, digital files that don’t translate and can’t be read by other systems, lost time spent on technological issues that <b>detract from what doctors are paid to do</b>, <b>which is treat their patients.</b> <i style="color: red;"><b>[And create <u>increased</u> <u>risk</u> that leads to maimed or dead patients - ed.]</b></i></blockquote><br /><b>"Familiar to anyone who deals with technology?"  </b>As in, say, mercantile/management computing that runs Walmart's stock inventory system, or the Post Office?  Is that an <a href="http://www.nizkor.org/features/fallacies/appeal-to-common-practice.html" target="_blank">appeal to common practice</a> of some type?  This brings to life my observation that there is an utter lack of recognition (either due to ignorance, or opportunism) that HIT systems are<b> not</b> business management systems<i> that happen to be used by clinicians</i>, they are virtual clinical tools (with all that implies) <i>that happen to reside on computers.</i><br /><br />How utterly amoral and <i>alien to the ethics of medicine</i> these attitudes are<i>.  No regulation of the technology is in place.  No systematic postmarket surveillance of patient harm or death is conducted.  </i><br /><br />Further, the <a href="http://hcrenewal.blogspot.com/2011/02/updated-reading-list-on-health-it.html" target="_blank">literature on health IT</a> is not entirely as optimistic and tolerant as the VT government.<br /><br />Is the VT government aware of that literature?<br /><br />Just a few specific examples - the National Research Council wrote that  "<span style="font-size: 100%;"><a href="http://www8.nationalacademies.org/onpinews/newsitem.aspx?RecordID=12572" target="_parent">Current Approaches to U.S. Healthcare Information Technology are Insufficient</a>", do not support clinician cognitive processes (then what, exactly, is health IT supposed to do?) and may result in harm. </span><br /><br />The <a href="http://hcrenewal.blogspot.com/2012/03/ecri-institutes-top-health-technology.html" target="_blank">ECRI Institute</a> indicates health IT systems are among the top ten technology risks in healthcare.<br /><br />Reports from the <a href="http://www.jointcommission.org/assets/1/18/SEA_42.PDF" target="_blank">Joint Commission</a>, <a href="http://hcrenewal.blogspot.com/2010/08/smoking-gun-internal-fda-memorandum-of.html" target="_blank">FDA </a>and <a href="http://healthit.ahrq.gov/AppendixB_HIT_Hazard_Manager_Beta_Test.pdf" target="_blank">AHRQ</a> indicate that <b>risk is known, but magnitude of risk unknown</b> ("<a href="http://hcrenewal.blogspot.com/2010/02/fda-on-health-it-adverse-consequences.html" target="_blank">tip of the iceberg</a>" per FDA).<br /><br />A report from <a href="http://hcrenewal.blogspot.com/2011/10/nist-on-ehr-mission-hostile-user.html" target="_blank">NIST</a> indicates that <b>usability is lacking</b> and promotes "<i>use error</i>" (technology-promoted error, as opposed to "<i>user</i> error").<br /><br />A report from <a href="http://hcrenewal.blogspot.com/2011/11/iom-report-on-health-it-safety-nix-fda.html" target="_blank">IOM</a> on safety of health IT states that:<br /><br /><blockquote class="tr_bq">... While some studies suggest improvements in patient safety can be made, others have found no effect. Instances of health IT–associated harm have been reported. However, little published evidence could be found <b>quantifying the magnitude of the risk.</b><br /><br />Several reasons health IT–related safety data are lacking include the <b>absence of measures and a central repository</b> (or <b>linkages</b> among decentralized repositories) to collect, analyze, and act on information related to safety of this technology. Another impediment to gathering safety data is <b>contractual barriers (e.g., nondisclosure, confidentiality clauses) that can prevent users from sharing information</b> about health IT–related adverse events. These barriers limit users’ abilities to share knowledge of risk-prone user interfaces, for instance through screenshots and descriptions of potentially unsafe processes. In addition, some vendors include language in their sales contracts and escape responsibility for errors or defects in their software (i.e., “<b>hold harmless clauses</b>”).<br /><br />The committee believes these types of contractual restrictions<b> limit transparency, which significantly contributes to the gaps in knowledge</b> of health IT–related patient safety risks. S-2.) </blockquote><br /><a href="http://www.hhs.gov/ohrp/archive/nurcode.html" target="_blank">Human subjects protections</a>?  Not needed here ... it's just one big, unconsented, happy $70 million medical experiment.<br /><br />What is not mentioned, and either deliberately ignored or lost on these politicians, is the effects of this turmoil on patient care in terms of <b>risk and adverse outcomes, and that being on the "bleeding edge" in this experimental technology is not desirable.  </b><br /><br />Slow, skeptical and cautious approaches are essential.<b>  Physician education on health IT risks and </b><b>patient informed consent </b><b>might be nice, too.</b><br /><br />It seems mass social re-engineering experiments are fine, even if the consequences include wasted resources the healthcare delivery system can ill afford, physicians being distracted by <a href="http://hcrenewal.blogspot.com/2011/01/maude-and-hit-risk-mother-mary-what-in.html" target="_blank">major defects</a>, and outcomes like <a href="http://hcrenewal.blogspot.com/2011/06/babys-death-spotlights-safety-risks.html" target="_blank">baby deaths</a>, <a href="http://hcrenewal.blogspot.com/2011/09/sweet-death-that-wasnt-very-sweet-how_24.html" target="_blank">adults suffocating at the bedside</a>, and <a href="http://hcrenewal.blogspot.com/2011/06/my-mother-passed-away.html" target="_blank">graveyards</a>.<br /><br />-- SS<br /><br />
